The Coastal Community Health and Social Care Team
The Adult Social Care service operating in the Coastal Locality forms part of the integrated Coastal Health and Social Care Team consisting of Adult Social Care, Community Nursing, Community Therapy, Intermediate Care, and Pharmacy. All disciplines within the team promote a person-centred and partnership approach to ensure we achieve the best possible outcomes for all adults, and their carers, within our locality.
The Coastal integrated team supports adults living across Teignmouth and Dawlish and surrounding areas. The office base is presently in Teignmouth Community Hospital. Currently a combination of office based and home working arrangements are in place, in line with Devon County Council guidance.
The Coastal Adult Social Care service includes a team of Social Care Assessors, Social Workers, Experienced Social Workers, Occupational Therapists and Team Managers. It is a busy team and case work is varied often requiring us to be flexible. The Disability Focus side of the team works closely with the Older People Focus part of the team to ensure the service we provide meets the needs of our local community.
We are looking for a Social Worker who is committed to a person centred strength based approach, alongside a commitment to promote independence.
As a Social Worker you will be required to undertake Section 42 safeguarding enquiries: lead on Mental Capacity Assessments and Best Interest processes including applications for community deprivations of liberties. You will need to be able to demonstrate a good knowledge of the Care Act and able to work in a strengths-based way to support individuals to reach their eligible outcomes and goals as well as strong risk assessment and risk management. You will be working alongside Social Care Assessors and Occupational Therapists and our health and voluntary sector colleagues following a multi-agency approach. You will be part of a supportive and proactive team.
Currently a combination of office based and home working arrangements are in place, in line with Devon County Council guidance. Due to the large geographic and rural nature of this area you will need to be able to drive and have access to transport.
Social Worker Career Pathway
We have implemented the first phase of a new Social Worker Career Pathway within our Adults Services, with our vacancies available at the Social Worker (Grade F) or Experienced Social Worker (Grade G) levels. Our Social Worker and Experienced Social Worker roles align with the national Professional Capability Framework (PCF) for Social Workers, which provides a robust structure for all local authorities to use as a way of standardising professional capabilities and provides a clear career pathway.
This is a fantastic opportunity to provide you with an achievable, clear pathway for your career; to enable you as a practitioner to reflect and identify your own practice and development on the PCF pathway.
If successful in being shortlisted for interview, your experience, skills and knowledge, will be assessed against the PCF domains and if found appointable, this will determine your appointment to either the Social Worker (Grade F) or Experienced Social Worker level (Grade G).
With on-going learning and support from your line manager, those appointed as a Social Worker will be able to evidence incremental learning and submit an application in due course for progression to the Experienced Social Worker (Grade G) via a portfolio of supporting evidence against the PCF framework.
