We have an exciting opportunitiy for a Social Care Assessor to join us in the Southern Devon Hospital Discharge Team.

About the Team
The Southern Hospital Discharge Team support people to return to the community following admission to hospital. We support in identifying the most appropriate care pathways to meet people’s needs, working closely with Multidisciplinary Teams.
The Team have a varied caseload supporting people who are currently inpatients as well as those who have recently been discharged; undertaking assessments to identify ongoing care and support needs.
We are an Integrated Service working closely with colleagues in the NHS, Torbay Adult Social Care, Short Term Services Teams, the Voluntary Sector as well as our community based colleagues.

About the role
This role covers the Southern Devon Hospitals (Torbay, Brixham, Newton Abbot, Dawlish and Totnes) but offers a mixture of both hospital based and home working. The Team have offices in Both Torbay Hospital and Newton Abbot Community Hospital.
You will work within the current discharge to assess model; in-reaching to hospital wards to complete onward care referrals, undertaking welfare checks for people post-discharge and the completion of community based Care Act Assessments in accordance with the Care Act 2014.
This role includes the completion of strengths based Care Act Assessments and Personalised Care Plans. You will also be responsible for the co-ordination of care, placements and ongoing social care support. You also will be supported to complete mental capacity assessments and assist in gathering information relating to safeguarding concerns.
You will be working as part of the wider Integrated Hospital Discharge Team consisting of Experienced Social Workers, Social Care Assessors and Discharge Liaison Nurses.
Please note that this position requires Saturday working on a rotational basis. Core Working Hours are 9am – 5pm (Monday to Thursday) and 9am – 4.30pm (Fridays).
About you
This opportunity would suit individuals looking to develop their skills, knowledge and practice experience within Adult Social Care as well as those considering onward professional development, such as undertaking a social work degree or apprenticeship.
You will ideally be experienced in Adult Social Care and have good verbal and written communication skills, with the ability to complete high quality assessments. You must be able to work both independently and within a team. The nature of this work requires you to be highly organised with strong recording skills; You will need to have the ability to prioritise referrals and manage competing demands simultaneously.
