Job Description
We are looking to recruit 3 skilled and experienced Support Workers to form a new community based service in South and West Devon.
We are keen to hear from you if you are you passionate about working alongside adults experiencing mental health difficulties and you have the skills to support them to develop their own strengths in terms of managing practical day to day living tasks. DPT’s Social Care provision is an integrated part of adult mental health services (alongside more treatment focussed parts) with a focus on areas like nutrition, personal care, managing home and finances, and exploring occupational and social access and opportunities.
Currently we commission much of this type of support through external care agencies. These roles will pilot a new in-house support work service. Support plans agreed with individuals will initially have a timeframe of 6-12 weeks with expectation that any long term support needs that need addressing after that period will be passed on to external care agencies. We will need you to help us measure the progress of the service and whether it is achieving the intended aims of bringing the assessment, delivery and review of support closer together to promote greater integration and communication; ensure efficiency in delivering support; and most importantly promote greater individual recovery.
The service will sit within, and be supported by, our existing experienced and settled DPT S&W Social Care Team.
We have 3 full time posts available located across South Hams & West Devon (1x Tavistock base and 1x Totnes base) and Teignbridge (1x Newton Abbot base) , with some flexible working from home. Please specify preferred location on application form. During the induction period the base for all 3 posts will be Totnes. Use of a Car and Driving License is essential.
Reflecting the partnership arrangements the posts are employed by Devon County Council but assigned to Devon Partnership NHS Trust.

We are looking for applicants who will be confident in working with people experiencing mental health difficulties, their families, carers, and other professionals. You will have an understanding of the importance of doing with, rather than doing for, and supporting people to enhance and draw on their own resources and those in their community. You will have experience of providing support to people with adult social care needs (Care Act 2014 eligibility outcomes), be an effective communicator both verbally and through written formats, be flexible, have good time management skills and be able to work under pressure. You need to be IT literate. You need to be open to learning and willing to develop your skills. You will be committed to equality and diversity, good customer service, be enthusiastic and look to achieving positive outcomes for the people we work with.
