Care Direct Plus, part of Adult Care & Health, has a vacancy for a Social Care Assessor in the Occupational Therapy Team based in Exeter, covering Eastern Devon. You will learn new skills and be supported as you gain experience through regular supervision from members of the team. Care Direct Plus assess and screen all referrals into Adult Social Care covering the Eastern Devon area. The Occupational Therapy team complete initial telephone assessments, give advice and information, arrange and undertake face to face visits when needed, or refer onto other teams when required. You will be working alongside Occupational Therapists and other Social Care Assessors.
You will be required to provide a person-centred service through assessment within the context of local government policy and legislation, including applying a strength based approach. You will be assessing the needs of people with a wide range of disabilities and health conditions both via telephone and face to face, with qualified Occupational Therapy support. You will manage a case-load and undertake lone working in a variety of settings. Your role will include enabling people to maximise their safety, independence and potential, as well as reducing risk to themselves and their carers.
Interventions will be determined by the Care Act 2014 eligibility criteria and the strength-based approach, but can include the provision of daily living equipment, arranging adaptations, giving advice and information, and on occasion arranging commissioned support. You will liaise with partner agencies as well as providing consultation, support and advice to a range of staff working within the remit of Care Direct Plus.

Covid-19: There will be the opportunity to work flexibly from home; with the Covid-secure work base remaining open for attendance as needed. In the future as the Council responds to a more flexible approach to meeting business needs, there will be continued opportunity to work from home for elements of the role. Additionally, under the current Adult Social Care Practice Guidance, face to face visits (with appropriate PPE) are only being completed where essential, non-deferrable and where all other options have been exhausted. The majority of assessments are currently being completed remotely (via telephone or via video-conferencing apps) to reduce the risk to both staff and clients.
