The Integrated Adult Social Care Team for Exmouth, Woodbury and Budleigh Salterton are looking to recruit two Social Care Assessors to support our work with vulnerable adults in the community.
We are looking for dynamic, proactive individuals who have a passion for best practice and delivering a service that supports those in need within our communities. Working in a strengths-based way, you will enable individuals to achieve maximum independence.
Using your knowledge and skills and the support of regular supervision and management support, you will assess needs using the Care Act 2014 as your legal framework, identifying areas of strengths and areas where support is needed from informal networks, voluntary sector, or commissioned care.
You will work closely alongside our colleagues in health in a multi-disciplinary approach to deliver holistic care and support planning.
With a robust training package and induction, you will be supported to carry out Mental Capacity Assessments, Assessments of Need and Support Planning including compiling evidence-based funding requests for senior management agreement. You will be encouraged to undertake training to support you in your role and DCC offer a wide range of training opportunities and career development pathways.
As part of an Integrated Adult Social Care Team, you will be working alongside colleagues from health and will play a significant role in professional meetings and conversations where you will represent Devon County Council and the individual you are working with. As a Social Care Assessor, you will also undertake planned reviews with individuals in receipt of services provided by DCC, ensuring the support is promoting independence and supporting individuals to reach their full potential.
There will be scope for working with young people who are in receipt of services in preparation for transitioning into Adult Services.
