Devon’s Bridges service (formerly Edge of Care) brings together a range of disciplines to work intensively and flexibly (including weekends and evenings) to provide timely and creative support to meet the complex needs of young people and their families and to prevent family breakdown.
The team works intensively and holistically with families, primarily in the home, to support change.
An introduction to the role and key responsibilities:
As an Adolescent Worker you will be working as part of the South locality team, working in partnership with other professionals to deliver timely support to young people and families. This includes developing and coordinating planned interventions using the Bridges Support Agreement and review process.
Utilising the Bridges practice model the Adolescent Worker takes the responsibility for coordinating Bridges interventions including the analysis and formulation of plans of support involving provision of data & reports and supporting social work plans.
You will provide intensive support to young people and families with multiple and complex needs and subject to a range of other statutory and voluntary interventions. Supporting young people with appropriate skilled interventions and practical support, both in response to crisis and for some families, as part of a longer-term support package.
You will need the skills to engage and build meaningful relationships with young people based on restorative principles and provide evidence-based interventions to support sustainable change. You will need to be persistent, tenacious and assertive, working with young people who may be resistant to support from a statutory agency.
What sorts of skills do you need?
You should be enthusiastic and motivated to work creatively with young people, parents and other professionals. This work will involve direct contact with young people and their families so will require good communication and engagement skills and an ability to work within a dynamic multi agency team.
You will need an ability to listen and use a restorative approach to support and challenge families to make positive changes in their lives.
