The role of the Social Care Assessor is to promote independence, identify care and support needs under The Care Act 2014 and work with vulnerable individuals to arrange support services that meet the identified need. Working alongside Social Workers, you will use a strength based approach and work with partner agenices and organisations in order to reduce the need for long term formal support.
As a Social Care Assessor within the Woodbury, Exmouth and Budleigh Adult Social Care Team (WEB) you would be expected to carry a mixed caseload of Older Adults, Physical Disability, Learning Disability and Mental Health. You will have opportunities to work with young people moving into Adult Services from Children and Family services and will also support vulnerable people living with alcohol and drug addiction.
You will be supported through a mixture of robust mandatory training, peer group support and induction and will receive regular supervision
You will work in Community based team that has a mixture of Social Workers, Occupational Therapists, Social Care Assessors and Care Coordinators. You will undertake face to face assessments within the community including residential and supported living placements and will be provided with PPE requirements as directed by Government Policy.
