We have an exciting opportunity for a Social Care Assessor to join us on a full time basis in the Torbay and South Devon Discharge Hub
About the Team
The Torbay and South Devon Discharge Hub is a joint venture for Devon County Council and Torbay and South Devon NHS Foundation Trust. The Discharge Hub Team coordinate and process referrals to support discharge from the Acute and Community Hospitals in Torbay and South Devon, West Devon, Derriford Hospital and The Royal Devon and Exeter Hospital.
The team support people to return to the community following admission to hospital. We support in identifying the most appropriate care pathways to meet people’s needs, working closely with Multidisciplinary Teams in both the hospital and the community.
The Team have a varied caseload supporting people who are currently inpatients and ensuring smooth transitions to community services following discharge from hospital. We are an Integrated Service working closely with colleagues in the NHS, Torbay Adult Social Care, Short Term Services Teams, the Voluntary Sector as well as our community based colleagues.

About the role
This is a full time role for 37 Hours per week.
This role offers a mixture of both office based and home working on a rotational basis. The Team’s office is located at St Edmunds Victoria Park Road Torquay TQ1 3QH.
Working within the current discharge to assess model, you will receive and triage referrals from hospital based colleagues, using your skills to determine which services are required to meet presenting health and social care needs. Liaising with partners both in the community and hospitals, you will coordinate discharge plans and complete onward referrals to community services to ensure continuity of care.
Please note that this position requires Saturday working on a rotational basis.
About you
This opportunity would suit individuals looking to develop their skills, knowledge and practice experience within Adult Health and Social Care as well as those considering onward professional development, such as undertaking a social work or occupational therapy degree or apprenticeship.
You will ideally be experienced in Adult Social Care and have good awareness of The Care Act 2014. You will have excellent verbal and written communication skills. You must be able to work both independently and within a team. The nature of this work requires you to be highly organised with strong recording skills; You will need to have the ability to prioritise referrals and manage competing demands simultaneously.
