The Coastal Intermediate Care Team
The Intermediate Care service operating in the Coastal locality forms part of the integrated Coastal Health and Social Care Team consisting of Adult Social Care, Community Nursing, Community Therapy, Intermediate Care, Pharmacy, and the voluntary sector. All disciplines within the team promote a person-centred and partnership approach to ensure we achieve the best possible outcomes for all adults, and their carers, within our locality.
The Coastal integrated team supports adults living across Teignmouth and Dawlish and surrounding areas. The office base is presently in Teignmouth Community Hospital. Currently a combination of office based and home working arrangements are in place, in line with Devon County Council and NHS guidance.
The Coastal Intermediate Care service includes a team of Occupational Therapists, Physiotherapists, Nurses, Paramedics, Support Workers, Co-ordinators and Social Care Assessors, that are led by the Intermediate Care and Therapy Lead. It is a busy team and case work is varied often requiring us to be flexible.
Social Worker:
This Social Worker role is a great opportunity for someone who wishes to further develop their skills, knowledge, and experience within the adult Health and Social Care sector. You will receive a comprehensive induction and be supported through regular 1:1 supervision, appraisal, regular team meetings and access to learning and development opportunities.
The successful candidate will demonstrate excellent communication skills, have the ability to manage and prioritise their own caseload, whilst working as a member of the integrated health and social care team.
The applicant needs to be flexible and able to meet the challenges of working in a busy community setting and there will be opportunities to gain experience in areas of special interest within the wider team.
You will support with the team by taking a variety of contacts and referrals, providing advice and information, co-ordinating immediate care and support arrangements, working in partnership with other agencies, and making onward referrals.
The role will require you to complete care and support needs assessments and develop and review personalised care and support arrangements, in accordance with the Care Act 2014.
You will support people to promote independence and apply an asset-based approach which seeks to optimise opportunities for people to reduce and delay their need for formal care and support.
You will be responsible for the co-ordination of care and support arrangements and will work closely with adults with care and support needs, their carers, care providers, and other services to support adults to achieve their outcomes and promote their safety and wellbeing.
You will be expected to complete mental capacity assessments and will be responsible for raising, and triaging, Safeguarding Concerns and to be the allocated practitioner for Safeguarding Enquiries.
We are looking for applicants who will be confident in working with people in sometimes difficult situations, their carers, and other professionals. You will have experience of working in a health or social care environment and have a good understanding of human needs. You will also have experience of undertaking assessments and eligibility criteria.
The successful candidate will be jointly supported by the Intermediate Care and Therapy Lead and the Team Manager of the Coastal Community Health and Social Care Team.
Full time and part time hours will be considered.

Currently a combination of office based and home working arrangements are in place, in line with Devon County Council guidance. Due to the large geographic and rural nature of this area you will need to be able to drive and have access to transport.

With on-going learning and support from your line manager, those appointed as a Social Worker will be able to evidence incremental learning and submit an application in due course for progression to the Experienced Social Worker (Grade G) via a portfolio of supporting evidence against the PCF framework.
