Care Direct Plus, part of Adult Social Care, has vacancies for a Social Care Assessor based in Exeter, covering Eastern Devon. You will be working in the Care Direct Plus team to offer advice and complete assessments for younger adults and older people, primarily via the telephone and other means of remote contact. You will be supporting people to retain their independence, taking a variety of referrals from the public and other professionals, providing advice and information, undertaking holistic assessments and support planning, working in partnership with other agencies, making onward referral, and reviewing services.
We are looking for applicants who will be confident in working with people in sometimes difficult situations, their carers, and other professionals. You will have experience of working in a social care or health environment and have a good understanding of human needs. Ideally you will also have experience of undertaking assessments. You will be able to manage your work individually as well as work as part of a team.
You should be an effective communicator both verbally and through written formats, able to prioritise workload, manage conflict effectively, be flexible and enthusiastic, have good time management skills, and be able to work under pressure. You need to be IT literate and willing to develop your skills. You will be committed to equality and diversity, to good customer service, and to achieving positive outcomes for vulnerable people.

The Social Care Assessor role within Care Direct Plus will be working a 5 day week on a staff rotation basis, over six days; Monday to Saturday, office hours. Saturday working attracts an enhanced rate. Currently staff are required to work on average one Saturday every 7 to 10 weeks, though this may be subject to change to meet the needs of the service. We are currently looking for internal applicants only.
Covid-19: whilst national or local restrictions remain in place there will be the opportunity to work flexibly from home; with the Covid-secure work base remaining open for attendance as needed. In future as the Council responds to a more flexible approach to meeting business needs, there will be continued opportunity to work from home for elements of the role.
