Part Time, 22.2 Hours per week (3 days)
We are looking for a Band 5 or Band 6 Occupational Therapist to join the West Exeter Community Health & Social Care Team.
This friendly team comprises of Experienced Social Workers, Occupational Therapists and Social Care Assessors. We work alongside our Urgent Community Response team, Physiotherapists, support workers and coordinators.
The aim is to support people in the community to remain in their own homes where possible. You will assess and plan a range of complex case work. Working across the city, you will have the opportunity to liaise with General Practitioners, the Community Nursing teams, Community Reablement teams as well as having regular contact with the voluntary sector and our local providers of Residential/ Nursing Care, Domiciliary care agencies and enabling agencies.
You will also work to avoid hospital admissions and provide timely assessments for people following hospital discharge. You will need to be able to drive and have access to your own transport.
You will be required to undertake outcome focused assessments which may involve co-ordination of placements and ongoing social care support.
You will be required to offer support and guidance around social care to the wider team. The nature of this work will require you to be highly organised and able to manage a constantly changing case load.
